Dr Dohrmann wants to ensure that all his patients are aware of the costs of treatment.  If you are scheduling surgery and there are likely to be gaps entailing out-of-pocket expenses to Dr Dohrmann, you will be given a statement regarding the estimated costs of treatment and the details of any gaps.  You will be asked to sign and return a copy of this statement provided you agree to the proposed schedule.  If you are unable to sign the proposal Dr Dohrmann will be happy to discuss it with you.
